DUI Registration Requirements
If you are enrolling in the DUI Program, you must have the following information with you at the time of enrollment:
1. Drivers License Number(s): Current number and number at the time of arrest, if not the same as current.
2. Photo Identification: Must be Florida ID if DUI was on Florida Driver License. This may be obtained from the Department of Motor Vehicles.
3. Complete DUI Arrest & Booking Report: Must include BAC Test Results or Refusal Affidavit and officer’s complete narrative. This may be obtained at the police station in the county which you were arrested.
4. Copy of Disposition: Court or Probation Order of DUI Arrest (if applicable). You can get this from your Probation Officer or the courthouse.
*** If the information on #3 and #4 are not available or has been destroyed because they’re old, we require proof of this in writing on their agency letterhead.
(DUI fees are regulated and set by the Florida Department of Highway Safety & Motor Vehicles and are subject to change annually)

Includes 12 hour classroom instruction and 1 hour evaluation
DUI Level 2………………………$480.00 EXACT MONEY ORDER ($495.00 with debit or credit)
Includes 21 hour classroom instruction and 1 hour evaluation
All DUI registration fees are payable at the time of enrollment.
*** Class placement for the DUI Level 1 Course is restricted to individuals who have only been arrested for one DUI in their lifetime. (In any state, anywhere since their very 1st license)
The registration includes completion of a written evaluation objective test, which must be completed at the time of registration. Please allow at least 1 hour to complete the registration process.
